The Changing Face of Root Canal Obturation: Past, Present and Future Directions

Abstract

Root canal obturation has shifted from a predominantly mechanical philosophy centred on three-dimensional compaction of gutta-percha towards a material-dependent strategy in which the sealer may contribute substantially to the bulk, interface and biological behaviour of the filling. This critical narrative review examines the historical development, present evidence and probable future trajectory of obturation, with particular attention to gutta-percha techniques, root canal sealer chemistry, hydraulic calcium silicate-based sealers, clinical outcomes, thermal compatibility and retreatability. Literature was selected through searches of PubMed, Europe PMC, PubMed Central, OpenAIRE, Crossref, DOAJ, Semantic Scholar and Google Scholar, supplemented by citation tracking, for evidence published from 1967 to 19 June 2026, while earlier conceptual material was considered when necessary. The evidence shows that cold lateral compaction remains a reproducible benchmark and that warm techniques can improve material distribution within irregular anatomy in laboratory models, but neither has demonstrated consistent long-term clinical superiority. Matched-taper single-cone obturation has become more credible with hydraulic calcium silicate-based sealers because these materials set by hydration, release alkaline and calcium-rich products and may form mineral deposits at interfaces. Nevertheless, the frequently used label “bioceramic” conceals substantial compositional heterogeneity, and favourable laboratory findings for mineralisation, adaptation or antimicrobial activity have not translated into a robust clinical healing advantage. Heat can alter sealer properties in a product-specific manner, while retreatment studies show persistent residual filling material regardless of sealer class. Contemporary clinical trials and systematic reviews increasingly suggest that obturation technique has a smaller effect on treatment outcome than infection control, baseline apical status, technical execution, coronal restoration and operator factors. The future of obturation should therefore be defined less by claims of universal material superiority and more by chemistry-specific evidence, clinically realistic testing, reversibility, long-term patient-centred outcomes and reproducible decision rules linking anatomy, sealer properties and technique.
