Accelerated Orthodontic Techniques and Periodontal Health: A Critical Comparative Review of Corticotomy and Piezocision in Aligner and Fixed Appliance Treatment

Abstract

Surgically facilitated acceleration of tooth movement has moved from a specialist adjunct to a widely promoted option in adult orthodontics, yet its periodontal consequences remain contested. Corticotomy-based procedures, including periodontally accelerated osteogenic orthodontics, and the minimally invasive piezocision technique both exploit a transient regional acceleratory phenomenon, but they differ in surgical access, capacity for simultaneous bone augmentation, and exposure of the marginal periodontium. Their comparative periodontal profile has been examined almost entirely in patients wearing fixed appliances, whereas the rapid displacement of adult treatment towards removable aligners has altered both the biomechanical environment in which surgery acts and the baseline periodontal risk against which any adjunct must be judged. This critical narrative review evaluates the strength, consistency, and interpretive limits of the evidence linking these two acceleration strategies to periodontal outcomes, and examines whether appliance type modifies the balance of benefit and harm. Sources were identified through structured searching of open scholarly indexes and citation tracking, with critical appraisal directed at design adequacy, outcome definition, follow-up duration, and the separation of surgical from prosthetic and biomechanical effects. The evidence supports three reasonably secure conclusions: acceleration is real but transient and largest during alignment and extraction-space closure; neither technique produces clinically important deterioration in conventional periodontal indices over the short to medium term; and grafted corticotomy, rather than acceleration itself, accounts for most demonstrated gains in alveolar bone thickness and keratinised tissue. Confidence is limited by small split-mouth trials, heterogeneous surgical protocols, follow-up rarely exceeding two years, near-absence of periodontally compromised participants, and a marked scarcity of studies in which surgery and aligner therapy are combined. Residual mucosal scarring, buccal crestal bone loss in thin phenotypes, and unverified long-term stability represent the principal unresolved risks. Research priorities include appliance-stratified randomised trials, standardised periodontal endpoints, and phenotype-based patient selection.
